I acknowledge that team members from the NewDay ministry team have voluntarily agreed to pray for me. I understand that this session is not a professional counseling session. I understand that the NewDay ministry team are to the best of their ability, doing what they can to help me achieve more freedom in my life. I understand that if I receive prayer ministry from the NewDay ministry team; the team is committed to respect the disclosed information, and will maintain a pastoral level of confidentiality. The information as needed, may be shared with those in training without revealing my identity. I also understand that the information may be shared with the ministry team. This information as given in writing, from me, may be shared with other persons involved in my healing process. The ministry team is a mandated reporter and is obligated by law to report abuse. I further state that I have voluntarily sought assistance of my own initiative and that I am under no obligation to accept or reject any of the advice or help that I might receive from the volunteer ministry team of NewDay. I agree to hold NewDay Church and its team members free from any and all liability, loss or damage of any kind that may arise as a result of assistance, which I have received, or from my involvement with NewDay Church. I understand that NewDay Church is a non-profit organization that makes no charges for its services. Our team offers biblical spiritual services to anyone who desires them regardless of the ability to pay. All efforts to build this ministry and train our team members are paid directly from donations given to the ministry. We therefore have a suggested donation of $75 per session.
